Social Worker: Full-Time MAIN FUNCTION: Provides direct social work
services to patients and families identified as having complex
needs. Works with the RN Case Manager to identify inpatients whose
social needs necessitate comprehensive psychosocial interventions.
Intervenes as necessary with patients seeking emergency care or
other ambulatory care services. Must be able to communicate
effectively with physicians, staff, patient/family and outside
agencies. This position must be able to handle multiple priorities
and be self-directed. It requires good organizational skills and
analytical thinking. RESPONSIBLE TO: Manager, Care Management
Minimum Qualifications: Bachelor’s degree in Social Work (BSW);
Master’s degree in Social Work (MSW) preferred . Current LSW or
LISW in the state of Ohio. Minimum of two years of experience in a
medical setting. Strong knowledge of social work principles,
patient advocacy, and community resources. Excellent communication,
interpersonal, and organizational skills. No written disciplinary
action in the last 12 months. Preferred Qualifications: Master’s
degree in Social Work (MSW) from an accredited program. Familiarity
with Meditech an electronic health record (EHR) and case management
software such as Careport. Training or certification in
trauma-informed care or crisis intervention. Position
Responsibilities: Conduct thorough psychosocial assessments of
patients and their families to identify needs and barriers to care.
Formulates and completes discharge plans for patients of all ages
in collaboration with physicians and other disciplines
participating in the care of the patient. Develop and implement
individualized plans of care in collaboration with health care
providers and community resources. Provide supportive counseling
and crisis intervention to support patients facing emotional,
social, or financial challenges. Advocate for patient access to
necessary services, including housing, financial assistance, and
mental health support. Maintain detailed documentation of patient
interactions, care plans, and progress in compliance with
regulatory standards. Collaborate with interdisciplinary teams,
including the RN CM, to coordinate care and ensure continuity of
services. Educate patients and families about available resources
and coping strategies to enhance their health outcomes. Assesses
for depression and anxiety using the PHQ-9 screening tool and the
GAD screening tool as indicated and refers to Master’s level social
worker when appropriate. Participate in supervisory case
consultation, staff meetings, and professional development
activities to stay current with best practices. 5 days a week.
Monday - Friday 0800-1630; Part of Saturday Rotation 40 hours per
week, salaried position
