found at ResponsibilitiesThe Clinical Partner (Medical Assistant)
will assist Harmony Cares Primary Care Physician, Nurse
Practitioners and Clinical Teams in the delivery of primary health
care and patient care management in a residential setting.
Essential Duties and ResponsibilitiesSupports mission, vision and
core values which include but not limited to delivering
compassionate support, attention and assistance to patients,
families and members of the teamSupport Patient Care through
Clinical Partnership with Nurse Practitioners, Physicians, and Care
Management TeamsCoordinates daily travel itinerary and transports
clinicians to scheduled appointmentsPrepares patients for
examination and treatment; May at times include preparing patients
for minor procedures Secures patient consent forms, records patient
care documentation accurately and in a timely mannerCoordinates
patient care as directed by Clinicians, company standards and
policiesRespects patient confidentiality at all times and treats
patients with courtesy and respectPerforms phlebotomy (venipuncture
only), EKG or any other procedure as requested by ClinicianLabels,
stores and properly packages specimens for lab deliveryAssists In
identifying and closure of quality gaps Documentation and updating
of patient care plans, as well as SDoH (Social Determinants of
Health) and other assessments within the patient chart or
electronic medical recordRequired to maintain compliance with CLIA
(Clinical Laboratory Improvement Amendments), OSHA, and HIPPAA
guidelinesPrepares and maintains supplies and equipment for
treatment (supply inventory, medical bag and car) (where
applicable) Ensure company vehicle is clean and proper periodic
maintenance schedule is followed (where applicable) Attends

Skills, and ExperienceHigh School graduate or GED
equivalentGraduate of an accredited Medical Assistant, Phlebotomy,
EMT, related medical education/training program or 1 year of
experience serving as a Medical Assistant or PhlebotomistCurrent
CPR certification that meets Company guidelinesKnowledge of medical
terminologyBasic computer literacy, including EMR, iPad, and GPS
experienceStrong interpersonal communication and customer service
skillsMust maintain a valid driver’s license and good driving
recordPreferred Knowledge, Skills and ExperienceCertified Medical
AssistantSix months of experience in patient care, preferably
geriatric careStrong Phlebotomy skillsDesire and ability to obtain
MA certification with 180 days of hireConditions of this role to be
aware of. . .Full range body motion including handling/lifting
patientsAbility to drive up to 150 miles daily in urban and/or
rural settingAbility to lift/carry items weighing up to 50
lbAbility to read and understand road mapsSit or stand for long
