Palliative Care for a per diem / prn job in Findlay, Ohio. Job
Description & Requirements - Specialty: Palliative Care -
Discipline: Nurse Practitioner - Duration: Ongoing - Shift: 9
hours, days - Employment Type: Per Diem Nurse Practitioner to join
our compassionate and collaborative team providing outpatient
hospice and palliative care services across Findlay, Ohio and
surrounding counties. Job Description: - Provide outpatient
palliative care services including home visits, extended care
facility consultations, telehealth, and clinic-based care. - Manage
6–8 patients per day based on location across a 9-county area in NW
Ohio. - Participate in delicate, goals-of-care discussions with
patients and families facing serious or terminal illnesses. -
Assess, plan, implement, and evaluate individualized care needs in
alignment with palliative care principles. - Collaborate with a
supportive interdisciplinary team including other NPs, physicians,
and administrative staff. - Participate in care transitions,
including determining when patients would benefit from continued
palliative care or hospice services. - Work closely with a
hospitalist team responsible for all patient admissions. - Document
patient encounters thoroughly and maintain excellent communication
with patients, families, and care teams. Compensation and Benefits:

Requirements: Board Certified NP - REQUIRED OH license (or willing
to obtain) - REQUIRED Experience as advanced provider in an acute
care setting or with palliative care and hospice - REQUIRED Clean
